Contact and Information Release 

Please have your Parent / Guardian Complete this section of the application

I hereby grant permission to NEED Access to College and Career Education Program staff to make contact with

my child and conduct a personal interview for the purpose of providing services. NEED may also make contact

with my child on school premises and as necessary for the purposes of screening and interviewing, as well as

ongoing support of his/her participation in the program.

I authorize NEED to obtain any needed information regarding my child from his/her school’s staff, including

academic and behavioral records, as well as conversations with teachers, counselors, and other administrative

staff.

I understand that any successful path to higher education includes a level of parental involvement and I,

therefore, understand that a NEED staff member may wish to contact me and meet with me for the purpose

of better helping my child. I will make myself available for such sessions and any parental workshops as much

as my schedule will permit.

I hereby grant permission to NEED, its employees, or agents to take photographs, videotape, or digital

recordings of my child throughout his/her duration in the NEED Access to College and Career Program.  I also

give consent for the images to be used in any and all media exclusively for the purpose of the NEED Program.
